Forms for MWR Customers

The Department of the Army has instituted a tracking system for Morale, Welfare and Recreation to better serve our customers. If you and your family members desire to use any of our MWR facilities, please complete the following application forms. Form #1 provides information on your household and Form #2 provides details on what interests you and your family. Gathering this data allows us to offer programs and services that meet your needs and enhance well-being for the Total Army Family.

Privacy Policy

The Directorate of Morale, Welfare & Recreation (DMWR) privacy policy is clear: We will collect no personal information about you when you visit our website unless you choose to provide that information to us.

Kids' Pages

We at DMWR are especially concerned about protecting children's privacy. We hope parents and teachers are involved in children's Internet explorations. It is particularly important for parents to guide their children when children are asked to provide personal information on line. DMWR will request no personal information from children.

For security purposes, and to ensure that this service remains available to all users, this computer system employs software programs to monitor network traffic, to identify unauthorized attempts to upload or change information, to cause damage, or to deny service to authorized users. Unauthorized attempts to upload information or change information on this service are strictly prohibited and may be punishable under the Computer Fraud and Abuse Act of 1986 and the National Information Infrastructure Protection Act.
